1. Applicant must complete and submit online application.
  2. Applicant must complete a telephone interview.
  3. Applicant must submit an updated resume.
  4. Applicant must complete professional questionnaire.
  5. Applicant must submit a minimum of three professional references, if a recent graduate (within 3 years), one of the references must be residency program director. (please see confidential professional references form).
  6. Applicant must disclose all claims history: past, pending and possible future claims they may know of.
  7. Applicant must submit of appropriate educational certificate as applicable. (Master of Science in Nursing (MSN), Doctor or Nursing Practice (DNP), Master in Physician Assistant Studies (MPAS), Medical School Diploma, Residency Certificate, Fellowship Certificate if applicable).
  8. Applicant must submit up to date appropriate training certificates as applicable. (BLS, PALS, NRP, ACLS, ATLS, ALSO etc).
  9. Applicant must submit copy of medical license per state they are applying to work in.
  10. Applicant must submit copy of Controlled Substance Registration (CSR).
  11. Applicant must submit copy of federal DEA registration.
  12. Applicant must sign and submit consent, authorization for release of information and release of liability form.
  13. Medical Resource Staffing LLC (MRS) will review and verify completeness and accuracy of the application including federal and state criminal background check, sexual abuse registry, verifying NPI number, validating educational and training history, validating work history, current state medical licenses, controlled substance registration certificate, federal DEA registration certificate, contact professional references and obtain written professional reference form, claims history and disciplinary actions history (Any claims history must be personally reviewed by the medical director).
  14. MRS will request written confirmation from potential providers prior to presenting them to potential clients.
  15. Entire checklist and application will be reviewed by the medical director with option for either approval or declination and signed by the medical director accordingly.
